Staff Summary
• ServicePoint is a web-based service coordination and homeless management information software administered by Lake County Community Development on behalf of the Lake County Coalition for the Homeless and the broader Lake County community.
• At its May 2018 meeting, the Lake County Board approved four legal documents for use by ServicePoint agencies, users and clients (as applicable):
o Data Services Agreement - to be used by all ServicePoint agencies;
o Client Authorization form - to be signed by all clients with information shared in ServicePoint;
o Business Associate Agreement - to be signed by all Health Insurance Portability and Accountability Act (HIPAA)-covered entities; and
o Business Associate Subcontractor Agreement - to be signed by vendors of HIPAA covered entities;
• This resolution delegates signature authority for ServicePoint legal documents to Lake County’s Community Development Administrator.

RESOLUTION
WHEREAS, ServicePoint is a web-based software package used by agencies throughout Lake County for service coordination and particularly as a homeless management information system (HMIS) required by the U.S. Department of Housing and Urban Development (HUD); and
WHEREAS, the U.S. Department of Housing and Urban Development (HUD) has empowered the Lake County Continuum of Care as managed by the Lake County Coalition for the Homeless to designate Lake County Community Development as the lead HMIS entity; and
WHEREAS, through its Housing and Community Development Commission (HCDC) the Lake County Board supports the community-wide implementation of the ServicePoint Referral Network; and
WHEREAS, proper implementation of all uses of ServicePoint requires legal documents including a Data Services Agreement, Client Authorization Form, Business Associate Agreement and Business Associate Subcontractor Agreement.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that these legal documents may be signed by the Lake County Community Development Administrator.
