We are recommending approval of the appointment and reappointment of the individuals listed in the suggested motion below. For those individuals recommended for appointment, we have performed the following verifications and there are no identified issues:
• Criminal background check
• Sex Offender registry check (CABS program providers only)
• IDFPR - license and controlled substance license
• OIG - gov’t healthcare programs: sanctions, restrictions or exclusions from participation
• AMA - medical school and residency
• ABMS - board certification
• NPDB - medical malpractice
• Hospital Privileges
