Northwestern Memorial HealthCare Award in the amount of $19,500 for the period of December 1, 2017 through November 30, 2018. The amount requested to be appropriated in LCFY18 is $19,500. These funds will be used for supplies and contractual services to complete a Community Heath Needs Assessment.
Suggested Motion
To accept the additional funding of $19,500 from Northwestern Memorial HealthCare. If deemed necessary, to authorize the Executive Director to execute an emergency appropriation or line item transfer to allow for expenditure of these funds.
