WHEREAS, Member Shari Bornstein has resigned from the Lake County Board of Health; and

WHEREAS, Shari Bornstein’s resignation has created a vacancy in the Lake County Board of Health; and

WHEREAS, your Chairman requests that this County Board confirm the appointment of Sylvia Johnson Jones as a member of the Lake County Board of Health for a term ending on June 30, 2020.

NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the appointment of Sylvia Johnson Jones as member of the Lake County Board of Health for a term ending June 30, 2020 be, and the same is hereby confirmed.
