2016 LCHD/CHC Annual Report - Carson

The Lake County Health Department and Community Health Center is required by statute to produce an annual report. This year’s report celebrates the 2016 accomplishments of the 2013-2016 Strategic Plan. It particularly highlights our work to address social determinants of health.
The report is divided into sections on health equity, prevention, coordination of care and workforce. It includes metrics that illustrate to what extent we met our strategic plan goals over the past year. It also looks ahead to how LCHD/CHC will be leading the conversation to promote the health and well-being of all who live, work and play in Lake County.
