The Lake County Health Department and Community Health Center presents the 2017-2019 Strategic Plan for board approval. Health department leadership has developed Health Impact in 3 (HI-3) goals to address the identified community health needs and for areas that are critical to the success and sustainability of the agency. With the social determinants of health as a lens that all actions are viewed through, the strategic plan focuses on: using data to drive decisions, quality services, proactive sustainability through improvements to the revenue cycle, improving outcomes for our patients through enhanced PCMH, and leading prevention and health education efforts across Lake County.

To approve the 2017-2019 Strategic Plan for the Lake County Health Department and Community Health Center as presented.
