Staff Summary

                     The Healthcare Foundation of Northern Lake County awarded a grant of $75,000 for the period November 21, 2016, through November 20, 2017, with possible annual renewals for up to a total of three years. 

                     The grant funds will be used to create an electronic referral network to be used by Lake County health and human service organizations.  The web-based software to be utilized for this purpose, called ServicePoint, is already in use at a number of area non-profits but its referral features remain, for the most part, inactive.  This grant will support bringing new and existing organizations online to a ServicePoint-facilitated referral network that will provide more meaningful and timely referrals between providers, provide agencies with a record of referral activity on the clients served and ultimately result in improved outcomes for the residents who access Lake County’s human services system. 

                     Key ServicePoint referral partners include the Lake County Health Department, Erie Family Health Center, Nicasa, and members of the Alliance for Human Services.

                     This ServicePoint grant funding in FY2017 will include $25,000 for a consultant who will provide expert analysis regarding the privacy concerns related to the network; $5,000 for a second consultant who will provide ServicePoint configuration support; $34,000 for salaries; and $11,000 for training, mileage and dues/subscriptions.

RESOLUTION

 

WHEREAS, Lake County Community Development has been approved for a $75,000 grant from the Healthcare Foundation of Northern Lake County to implement a ServicePoint-facilitated electronic referral network to be used by Lake County health and human service organizations; and

 

WHEREAS, the Strategic Plan of the Lake County Board includes the strategy to foster partnerships and teamwork among stakeholders and leverage resources to address the evolving demand for health and human services; and

 

WHEREAS, additional funding is necessary to augment ServicePoint funding granted to Lake County by the U.S. Department of Housing and Urban Development for Homeless Management Information Services; and

 

WHEREAS, the Healthcare Foundation of Northern Lake County has approved the first year of a potential three-year grant to assist with this need, with matching requirements met by existing grants from the U.S. Department of Housing and Urban Development; and

 

WHEREAS, an emergency appropriation is necessary to formally accept this $75,000 grant agreement from the Healthcare Foundation of Northern Lake County into revenue account 740-2950020-45330-000-000- 000-00000; and $30,000 for contractual services (expense account 2950020-79940-000-000-00000), $34,000 for salaries (expense account 2950020-51110-000-000-00000), $7,500 for trips and training (expense account 2950020-71500-000-000-00000), $2,500 for mileage reimbursement (expense account 295020-71450-000-000-00000) and $1,000 for dues and subscriptions (295020-71810-000-000-00000).

 

NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois that the acceptance and execution of a $75,000 grant agreement through November 20, 2017, by the Healthcare Foundation of Northern Lake County grant to Lake County Community Development is hereby approved; and

 

BE IT FURTHER RESOLVED, that Lake County Community Development shall serve as project director for the program and maintain accurate records of all necessary grant compliance requirements.
