R E S O L U T I O N
WHEREAS, Lake County holds a tract of land as Trustee (COLT Property) under 35 ILCS 200/21-90, which is described in the attached "Utility Line Easement Agreement," located at an address commonly known as 25635 U.S. Highway 45, Mundelein, Illinois, 60060; and
WHEREAS, this tract of land has a PIN of 15-06-203-019; and
WHEREAS, this tract of land is immediately adjacent to the parcel on which Transitional Care of Lake County (TCLC), also known as Transitional Care of Mundelein, intends to build the replacement facility for Winchester House; and
WHEREAS, in order for the replacement facility for Winchester House to be built, it requires looped water line connections from two separate Village of Mundelein water mains; and
