SAMHSA Primary-Behavioral Health Care Integration Grant Approval - Dorn

The Lake County Health Department and Community Health Center was awarded a grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) in the amount of $400,000. This was a competitive grant with other agencies across the country. We applied for this grant to support and enhance the integration project we started in 2015 at the Libertyville Health Center. The budget period is from September 30, 2016-September 29, 2017.
Funds will be used to demonstrate improved health outcomes among people with severe mental illness by adding supportive services to the integrated behavioral health and primary care health home at the Libertyville (Peterson Road) location. In addition to the psychiatry, therapy and general medical care already available at the site, patients will have access to supportive services including peer support, substance abuse counseling, tobacco cessation counseling, care coordination, case management and nutritional counseling. In addition to funding 4.55 FTE positions, grand funds will also be used to purchase medical supplies, transportation, and training for staff to provide evidence-based treatment.
The project period for the grant is four years with expected future payments of $362,064 in year two, $375,849 in year three and $388,233 in year four. The impact of these services, using both clinical and financial measures, will be tracked and evaluated to support the sustainability of the care model.
