Body

The Illinois Project for Local Assessment of Needs (IPLAN) is a series of planning activities conducted by our health department with assistance from the Live Well Lake County Steering Committee resulting in an organizational capacity assessment, a community health needs assessment, and a community health improvement plan.  IPLAN completion demonstrates a local health department’s commitment to providing the three core public health functions and is also a requirement of the Part 600 Certified Local Health Department Code.  The IPLAN requires submittal of a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP) to the Illinois Department of Public Health by September 2016.

 

The CHA utilizes both quantitative and qualitative methods to collect and examine health status indicators and provide an understanding of health in a community.  Risk factors that impact health, mortality, morbidity, the capacity of the local public health system, quality of life, community assets, social determinants of health, internal and external forces of change, and health inequities were identified to determine the community’s key health issues.

 

The CHIP is a five year, systematic and focused plan that will lead to implementation of interventions to address our four identified public health priorities: cardiovascular disease and hypertension, obesity, behavioral health, and diabetes.  These four priorities are based on the results of community health assessment activities and the community health improvement process with the vision of “Achieving the highest level of health and wellness for all in Lake County.”  The plan is intended to engage and empower partners to implement coordinated efforts, and evaluate actions to assure success.
