Referral Laboratory and Phlebotomy Services - Nordstrom

The Lake County Health Department and Community Center is seeking a qualified supplier to provide Referral Laboratory and Phlebotomy Services. A legal notice was published in The News Sun soliciting Requests for Proposals (RFP 16174). The RFP was also posted to the LCHD/CHC website and emailed to five suppliers. Sealed proposals were received from two suppliers and opened in the Materials Management office on June 23, 2016. An evaluation committee comprised of LCHD/CHC staff reviewed and evaluated the proposals and interviewed both suppliers. Based on the evaluation criteria within the RFP, the committee determined the response from Quest Diagnostics of Wood Dale, IL, to be the preferred submittal which best meets the needs of the LCHD/CHC.
The estimated amounts are calculated based on historic volumes provided in the RFP. Based on Quest Diagnostics' response, the total estimated annual charge for the service is $1,030,093 during the initial two (2) year period, with three (3) additional one (1) year renewals subject to acceptable performance by the contractor. Prices will remain firm/fixed for years one (1) and two (2). The supplier must submit a written request 60 days in advance of the annual contract period for any future price increases after the initial two (2) year contract period.
The stated annual amount is helpful for comparing proposals, but does not represent the actual cost to LCHD/CHC. Much of the cost is covered by other parties, including Medicaid, Medicare, and insurance. Costs to be covered by LCHD/CHC will be paid from multiple management centers in amounts currently budgeted totaling $681,906.
