Staff Summary

* The Healthcare Foundation of Northern Lake County awarded $100,000 for a jail diversion and health engagement program to support planning and development efforts for the period November 2015 to November 2016.
* The March of Dimes awarded $15,000 for the March of Dimes Prevention Grant which will offset existing staff and program supplies for the period March 7, 2016 through March 6, 2017.
* The Illinois Department of Public Health awarded $38,374 for the Chronic Disease and School Health grant which will offset existing staff and program supplies for the period of July 1, 2015 through June 30, 2016.
* The Illinois Department of Human Services awarded $48,542 for the Child and Family Connections grant which will be used to offset existing staff costs and one laptop for the period of July 1, 2015 through June 30, 2016.
* The Illinois Children's Healthcare Foundation awarded $271,000 to support the construction of a new community health center facility in Zion, Illinois.
* The Health Resources and Services Administration awarded $325,000 to expand access to medication-assisted treatment which will be used to add 2.5 FTE new staff, the projected program income will be $140,397.

RESOLUTION

WHEREAS, the Lake County Health Department and Community Health Center has been awarded grant increases delivered by our Primary Care and Population Health Service Areas; and

WHEREAS, these dollars will provide a legitimate increase in services to Lake County with no impact on local taxes; and
