* The two year contract, with three additional one year renewals, will cost the County an annual amount of $64,500.

RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing
Ordinance, proposals were called for by publication for Insurance Broker Services for Lake County; and
WHEREAS, subsequent to said publication, requests for proposals were extended to 15 vendors; and
WHEREAS, sealed proposals were received from three vendors and opened in the County Building, Waukegan, Illinois, on March 31, 2016 at 2:00 p.m.; and
WHEREAS, an evaluation committee, consisting of representatives from the Human Resources Department and States Attorney's Office, held interviews with the top two firms; and
WHEREAS, based on the criteria set forth in the Request for Proposal, the evaluation committee selected Mesirow Insurance Services, Chicago, Illinois, as the most favorable proposal for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that that the Purchasing Agent is authorized to enter into a contract with Mesirow Insurance Services, Chicago, Illinois, for Insurance Broker Services for Lake County for a two year period, plus renewals, in the estimated annual amount of $63,500 charged to 206-1440010-72160.
