RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing
Ordinance, proposals were called for by publication for Dental Insurance for Lake County; and
WHEREAS, subsequent to said publication, requests for proposals were extended to 15 vendors; and
WHEREAS, sealed proposals were received from three vendors and opened in the County Building, Waukegan, Illinois, on May 5, 2016 at 2:00 p.m.; and
WHEREAS, an evaluation committee, comprised of representatives from the Human Resources Department and Lake County Forest Preserve Human Resources Department, held interviews with the top two firms; and
WHEREAS, based on the criteria set forth in the Request for Proposal, the evaluation committee selected Delta Dental, Naperville, Illinois, as the most favorable proposal for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that that the Purchasing Agent is authorized to enter into a contract with Delta Dental, Naperville, Illinois, for Dental Insurance Services for Lake County for a two year period, plus renewals, in the estimated annual amount of $2,079,844.50 charged to 510-1430010-74030.
