* The carpet and resilient tile work in this request for proposal generally includes material and installation of all carpet and resilient tile flooring for all phases of the Courthouse Expansion Project.
* The Lake County Board has authorized Clark Construction Group, LLC to proceed with a Guaranteed Maximum Price (GMP) for the Infrastructure & Interior work pursuant to their contract for Construction Manager at Risk (CMAR) services.
* A request for proposal was issued for carpet and resilient tile work on May 8, 2015.
* Sealed proposals were received on June 4, 2015 from three nonlocal vendors.
* In accordance with the evaluation criteria established in the RFP, staff reviewed and determined that the proposal submitted by Mr. David's Flooring International, LTD of Chicago, Illinois, is the most qualified and favorable proposal for Lake County.
* This resolution awards the contract to Mr. David's Flooring International, LTD and assigns the contract to Clark Construction Group LLC to manage as part of the GMP for this work.

RESOLUTION
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, a requests for proposal was called for by publication for Carpet and Resilient Tile work for Lake County Courthouse Expansion Project; and
WHEREAS, sealed proposals were received from three vendors at the Lake County Administrative Building, Waukegan, Illinois, on June 4, 2015, at 2:00 p.m.; and
