The Illinois Department of Public Health, PHEP LHD Ebola Grant Award in the amount of $104,590 for the period of July 1, 2015 through June 30, 2016.  The amount requested to be appropriated in LCFY15 is $104,590.  These funds offset existing staff, general supplies and mileage.

 

The Illinois Department of Public Health, SFY16 Chronic Disease and School Health Grant Award in the amount of $90,933 for the period of July 1, 2015 through June 30, 2016.  The amount requested to be appropriated in LCFY15 is $38,000.  The difference is budgeted in LCFY16.  These funds offset existing staff and advertising.

 

The Illinois Department of Public Health, Vector Surveillance Grant Award increase in the amount of $22,925 for the period of April 1, 2015 through March 31, 2016.  The amount requested to be appropriated in LCFY15 is $15,285.  The difference is budgeted in LCFY16.  These funds are for mosquito larvacides.

 

Advocate Health Care, Mobilizing for Action through Planning and Partnerships (MAPP) Donation in the amount of $9,200.  The amount requested to be appropriated in LCFY15 is $9,200.  These funds are for printing services.

 

March of Dimes, Becoming a Mom Grant Award in the amount of $8,000 for the period of September 1, 2015 through August 31, 2106.  The amount requested to be appropriated in LCFY15 is $8,000.  These funds offset existing staff and program supplies.

 

HIV National Testing Day and World Hepatitis Day Exhibit fees in the amount of $6,000.  The amount requested to be appropriated in LCFY15 is $6,000.  These funds are for program supplies and contractual services.

 

The United Way, Success By 6 Home Visiting Grant Award increase in the amount of $5,000 for the period of July 1, 2015 through June 30, 2016.  The amount requested to be appropriated in LCFY15 is $5,000.  These funds offset existing staff and medical supplies.

 

MDRC, Maternal Infant Home Visiting Program Evaluation Strong Start funding in the amount of $3,750 for the period of December 1, 2015 through September 30, 2015.  The amount requested to be appropriated in LCFY15 is $3,750.  These funds offset existing staff.

 

Suggested Motion

To accept the additional funding of $250,398 of which we are requesting an emergency appropriation of $189,825 for FY15 from the Illinois Department of Public Health, Advocate Health Care, March of Dimes, HIV Exhibit fees, United Way and MDRC.  If deemed necessary, to authorize the Executive Director to execute an emergency appropriation or line item transfer to allow for expenditure of these funds for enhanced programming in Population Health Services.
