* The current contract to provide pre-employment physicals and other medical services for Lake County is expiring.
* A Request for Proposal (RFP) was issued to identify a qualified firm to provide pre-employment physicals and other medical services for Lake County.
* A RFP was extended to eight vendors and proposals were received from two vendors.
* In accordance with the evaluation criteria established in the RFP, the selection review team made up of representatives from Human Resources, Risk, Division of Transportation, Health Department and Lake County Sheriff's Office reviewed and determined that the proposal submitted by Advocate Medical Group is the most qualified and favorable proposal for Lake County.
* The contract authorizes a two year agreement with three additional one-year renewals that will cost the County an estimated annual amount of $112,645.

RESOLUTION
WHEREAS, in accordance with chapter 33.051 of the Lake County Purchasing Ordinance, Proposals were called for by publication for pre-employment physicals and other medical services for Lake County ; and
WHEREAS, subsequent to said publication, a Request for Proposal (RFP) was extended to eight vendors; and
WHEREAS, sealed proposals were received and opened from two vendors in the County Building, Waukegan, Illinois, on Thursday, August 27, 2015, at 2:00 p.m.; and
WHEREAS, in accordance with the evaluation criteria as established in the RFP, the Selection Review team determined that the proposal submitted by Advocate Medical Group of Chicago, IL is the most qualified and favorable proposal for Lake County.
