Staff Summary
* The Health Care Foundation of Northern Lake County awarded a grant for a Care Coordination Program in the amount of $95,000 for the Metropolitan Chicago Healthcare Council Health Information Exchange one time participation fee.
* The Illinois Department of Public Health awarded $8,400 for the Category C grant which will be used to offset existing staff costs for the period January 1, 2015 through December 31, 2015.
* The National Association of County and City Health Officials awarded $3,500 for the Medical Reserve Corps contract which will be used for MRC staff attending conferences for the period January 1, 2015 through July 31, 2015.
* The Illinois Department of Public Health awarded $3,600 for the Child and Adult Care Food Program contract which will be used to offset existing staff costs for the period October 1, 2014 through September 30, 2017. The amount requested to be appropriated in LCFY15 is $960.
* The Illinois Department of Public Health awarded $1,143 for the Ebola Direct Active Monitoring grant in the amount of $1,143 which will be used to offset existing staff costs for the period December 22, 2014 through June 30, 2015.
* The Coalition for Healthy Communities gave a donation for Life Skills in the amount of $1,000 which will be used for supplies for stress prevention to 5th and 6th graders in Zion for the period December 1, 2014 through December 31, 2014.
* The Illinois Department of Public Health awarded $90,933 for the Chronic Disease and School Health grant which will be used to offset existing staff and advertising costs for the period of January 1, 2015 through June 30, 2015. The amount requested to be appropriated for LCFY15 is $46,496.
