R E S O L U T I O N

WHEREAS, Lake County, by and through its Division of Transportation, expects to process over 10,000 storm sewer ticket screening and locate requests from JULIE in 2015 which exceeds our annual estimate; and

WHEREAS, this Lake County Board, at its meeting of May 8, 2012, awarded a contract to USIC Locating Services, Inc., Indianapolis, Indiana, for storm sewer ticket screening and locate services required to comply with "The Illinois Underground Utility Facilities Damage Prevention Act" for the time period May 15, 2012, thru December 31, 2012; and

WHEREAS, said contract contains a clause providing for the extension of said contract for four additional one year periods starting January 1 of each year; and
