The Illinois Department of Public Health FY15 Category C Grant Award in the amount of $8,400 for the period January 1, 2015 through December 31, 2015. The amount requested to be appropriated in LCFY15 is $8,400. These funds are for the participation in a statewide collaboration project regarding HIV Prevention with Black and Latino Men and Transgender Persons who have Sex with Men. The aim of the project is to link HIV positive individuals with medical care and retain them in comprehensive care services. These funds offset existing staff.

The National Association of County and City Health Officials Medical Reserve Corps Contract in the amount of $3,500 for the period January 1, 2015 through July 31, 2015. The amount requested to be appropriated in LCFY15 is $3,500. The funding will go towards MRC staff attending the MRC conference and the Illinois Emergency Service Management Association conference.

The Illinois Department of Public Health Child and Adult Care Food Program Contract in the amount of $3,600 for the period October 1, 2014 through September 30, 2017. The amount requested to be appropriated in LCFY15 is $960. The remainder will be budgeted in the appropriate fiscal years. These funds are to provide inspections for approved Child and Adult Care Food Program sites in order to decrease disease and prolonged health issues that may occur among users of these type establishments. These funds offset existing staff.

The Illinois Department of Public Health Ebola Direct Active Monitoring Grant in the amount of $1,143 for the period December 22, 2014 through June 30, 2015. The amount requested to be appropriated in LCFY15 is $1,143. These funds are to support local health department activities in Active Monitoring or Direct Active Monitoring for persons under investigation for Ebola Virus Disease. These funds offset existing staff.
