Body
Every five years, the Lake County Health Department is required to conduct an IPLAN.  The completion of IPLAN fulfills most of the requirements for Local Health Department certification under Illinois Administrative Code Section 600.400: Certified Local Health Department Code Public Health Practice Standards.
 
The essential elements of IPLAN are:
 
·      An organizational capacity assessment
·      A community health needs assessment
·      A community health plan, focusing on a minimum of three priority health problems
 
Our IPLAN is due to the Illinois Department of Public Health in September 2016.
 
The Live Well Lake County steering committee is currently meeting on a monthly basis to guide the process.  Similar to 2011, we are using the MAPP tool (Mobilizing for Action through Planning and Partnerships) to conduct our community health assessment and community health improvement plan.  By conducting the four MAPP assessments, the steering committee and other Lake County residents will be able to prioritize community needs, set focused objectives, and create an implementation plan to improve the health of Lake County communities.  
 
Our first assessment, the Local Public Health System Assessment, will occur on June 18, 2015 at Rosalind Franklin University of Medicine and Science.  This assessment will allow us to identify at what level the 10 essential public health services are being provided to our community. Over 50 community members will participate on one of five focus groups.  Other assessments will provide additional health data about Lake County that will allow us to prioritize community health needs.  The assessment reports will be completed in November 2015.
