On Friday April 10, 2015, the Health Department submitted our final evidence to the Public Health Accreditation Board, thus concluding a yearlong documentation collection phase. Over 2,000 documents were reviewed and analyzed with only the best being selected for submittal.
To date, we have made several agency improvements:
· A measurable strategic plan linked to the Community Health Improvement Plan (CHIP) and to programmatic performance measures;
· An agency wide performance management system;
· Standardized quality improvement efforts in each Health Department program;
· Updated policies and procedures.
