On January 5, 2015, the Healthcare Foundation of Highland Park awarded Friends for Health a total of $577,231 in grant funding.  Friends for Health is a 501c3 organization that partners with the Lake County Health Department and Community Health Center (LCHD/CHC) to support services at the North Shore Health Center (NSHC).  The management of funds and implementation of proposed activities are overseen by the LCHD/CHC.  Therefore, the receipt of funding needs to be approved by the Governing Council.
 
LCHD/CHC anticipated an award from the Healthcare Foundation of Highland Park for Fiscal Year 2015 and a total of $556,062 had already been allocated in the FY15 budget.  This agenda item will approve the additional $21,169 that was included in the grant award.
 
The grant will specifically fund the continuation of three programs at the North Shore Health Center:
 
1) Specialty Care, Diagnostic Testing and Medication program:  This program supplements primary care services delivered at the NSHC by helping patients access specialty care and other supplemental services.
 
2) Health and Wellness Program:  This program promotes healthy lifestyles among NSHC patients.  It specifically addresses childhood and adult obesity.  The Health and Wellness program also funds community health workers called "promotores de salud" (health promoters) who work in the community to promote healthy living.
 
3) Diabetes Prevention and Treatment Program:  This program provides education and assistance to help individuals living with diabetes manage their disease. Patients are able to meet with a registered dietitian.  They also receive specific diabetes management medical visits where the physician is able to spend additional time consulting with the patient and discussing the specifics of their condition.
 
The $21,169 allocated through this agenda item will be dedicated to Specialty Care, Diagnostic Testing and Medication.  LCHD/CHC does not include any revenue projections as part of this grant.
 
LCHD/CHC first received financial support from the Healthcare Foundation of Highland Park (HFHP) in 2002 and, for the last seven years, funding has been dedicated to the programs outlined above.  From 2014 to 2015, LCHD/CHC reduced the requested amount from HFHP by 6% (from $615,000 to $577,231).  This reduction is based on a refinement of estimated costs and is not expected to decrease the services delivered.
 
This program supports the LCHD/CHC Strategic Plan by addressing two specific strategic priority areas:  Coordination of Care:  Access to a Medical Home and Behavioral Health; Coordinated Network of Health and Human Services AND Emphasis on Prevention/Access to Prevention and Wellness.  Both the specialty care and diabetes management program reduce barriers to care and covered services.  The Health and Wellness program specifically addresses the strategic objective to reduce the percentage of adults and children in Lake County who are overweight or obese.
 
The grant period for the award is January 1, 2015 - December 31, 2015.
 
Suggested Motion
To approve the grant funding of $21,169 from the Healthcare Foundation of Highland Park.  Friends for Health is the award recipient but funds will be managed and activities will be implemented by the Lake County Health Department and Community Health Center.  These funds will be used to support the Specialty Care, Diagnostic Testing and Medication program at the North Shore Health Center.
