An agreement was entered by the Lake County Board of Health and the Lake County Community Health Center Governing Council on the 27th day of May, 1992 to provide for operation, administration, and provision of certain primary care services in Lake County, Illinois.
Pursuant to the Section 330 of the Public Health Service Act Grant operating a Community Health Center from the U.S. Department of Health and Human Services, the Board of Health wishes to give support to the Governing Council, established as an extension of the Lake County Health Department as a public, not-for-profit entity, and recognize its powers, privileges, and functions.
