Staff Summary
* The current contract for adult residential co-occurring disorder treatment has expired.
* There is a need to establish a contract with a vendor to provide adult residential co-occurring disorders treatment, recovery home services, and recovery coaching services for the Lake County Adult Probation and Psychological Services Divisions of the Administrative Office of the Nineteenth Judicial Circuit.
* Lake County extended a request for proposal (RFP) to eight (8) vendors and received one (1) proposal.
* In accordance with the evaluation criteria as established in the RFP, the Adult Probation Services Division reviewed and determined that the proposal submitted by Haymarket Center is acceptable.
* This contract will be paid from the Probation Services Fee Fund, account #250-3210010-72040 and account #101-3200025-72040; total contract costs will not exceed the annual budget amount for this service of $400,000 per year.

RESOLUTION

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, Proposals were called for by publication for Adult Residential Co-Occurring Disorder Treatment for the Adult Probation Services Division of the Administrative Office of the Nineteenth Judicial Circuit Court; and

WHEREAS, subsequent to said publication, request for proposals were extended to eight (8) vendors; and

WHEREAS, a sealed proposal was received and opened from one (1) vendor in the County Building, Waukegan, Illinois, on Thursday, December 11, 2014, at 2:00 p.m.; and
