Body
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, Request for Qualifications were called for by publication for the Public Health Programs Evaluation Consultant to perform a systematic review of programs and services provided by the Lake County Health Department and Community Health Center.
WHEREAS, subsequent to said publication, Request for Qualifications were extended to 207 vendors; and
WHEREAS, sealed proposals were received from three vendors and opened in the County Building, Waukegan, IL on February 17, 2012 at 2:00 p.m.; and
WHEREAS, based on the criteria set forth in the Request for Qualifications for Public Health Programs Evaluation Consultant, a panel consisting of staff from the Health Department, County Administrator's Office and Financial and Administrative Services recommend contracting with Health Management Associates.
NOW THEREFORE, be it resolved by this County Board of Lake County, Illinois, that the Purchasing Agent is authorized to finalize revisions as may be necessary and execute the agreement with Health Management Associates in an estimated amount not to exceed $169,189 charging the costs thereof to account # 101-1102040-71150-000-000-000-11070.
