· Lake County desires to enter into a contract for physical and occupational therapy and speech pathologist services.
· Request for proposals were sent to 207 vendors with four firms submitting proposals.
· The Comprehensive Group, is recommended as the firm that best meets the needs of the rehabilitation residents at Winchester House.
· The resolution authorizes the Purchasing Agent to execute a two-year agreement with three additional one year renewal options for an estimated annual amount of $744,000.
· The money spent on the contract is recouped through Medicare billing.

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, RFP proposals were called for by publication for the Physical & Occupational Therapy and Speech Pathologist Services; and
WHEREAS, subsequent to said publication, Request for Proposals were extended to 207 potential vendors; and
WHEREAS, sealed proposals were received from four vendors and opened in the County Building, Waukegan, IL on January 24, 2012 at 2:00 p.m.; and
WHEREAS, based on the criteria set forth in the Request for Proposal for Physical & Occupational and Speech Pathologist Services it has been determined that the proposal submitted by The Comprehensive Group a HealthPro Company is the most favorable for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent is hereby authorized to enter into a contract with The Comprehensive Group a HealthPro Company in the estimated annual amount of $744,000 for the Physical & Occupational Therapy and Speech Pathologist Services
charging the costs thereof to account # 218-4102010-72870.
