Staff Summary
· The current total cost to purchase certified copies of death and fetal death certificates is $10.00 for the first copy and $6.00 for each additional copy.
· The aforementioned charges include a fee of $2.00 per copy issued that goes to the State of Illinois Death Certificate Surcharge Fund.
· As required by Public Act 97-0679, an additional $2.00 fee is to be collected and deposited into the Cemetery Oversight Licensing and Disciplinary Fund, and results in additional State mandated fees totaling $4.00 per copy.

WHEREAS, the current total cost to purchase certified copies of death and fetal death certificates is $10.00 for the first copy and $6.00 for each additional copy. This includes a fee of $2.00 per copy issued for the State of Illinois Death Certificate Surcharge Fund (DCSF).
WHEREAS, an additional State of Illinois fee of $2.00 is to be collected beginning July 1, 2012 for the issuances of certified copies of death and fetal death certificates.
WHEREAS, PA 97-0679 requires this additional state fee to be deposited into the Cemetery Oversight Licensing and Disciplinary Fund (COLDF) and used by the Illinois Department of Financial and Professional Regulation (IDFPR).
WHEREAS, beginning July 2012, the total additional fees will be increased to $4.00 per copy.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the total fee for purchasing a certified copy of death or fetal death certificate will be $12.00 for the first copy and $8.00 for each additional copy effective July 1, 2012.
