Request to Apply for the Health Resources and Services Administration Expanding Nutrition Services Grant Funding - Kritz

In accordance with the Lake County Health Department and Community Health Center’s Grant Management Lifecycle Policy, Governing Council approval is required before applying for Health Resources and Services Administration funding associated with the Federally Qualified Health Center.

 

Lake County Health Department and Community Health Center is requesting approval to apply for the Health Resources and Services Administration Expanding Nutrition Services grant funding for the period of December 1, 2026, through November 30, 2028. The request includes $324,283.90 for the first year and $307,579.76 for the second year, with a total request of $631,863.66 in funding.

 

If awarded, the funding will expand nutrition services to help prevent and manage diabetes, prediabetes, obesity, hypertension, and other nutrition-related chronic conditions. The proposed project will increase access to Medical Nutrition Therapy, expand preventive nutrition services for patients five and older, strengthen services at the Round Lake School-Based Health Center, and improve outreach and follow-up for patients at elevated risk. The funding will be used to hire one registered dietitian, two clerk translators, and a clerk translator supervisor; provide recruitment incentives for two dietitian positions; and offer specialized obesity training for the dietitian team.
