Staff Summary
· 2012 Concrete Pavement Patching: Change Order No. 1 and Supplemental Appropriation.
· Each year, the County undertakes patching of its concrete pavements to preserve surface quality, including repairing damaged manholes, catch basins, curbs, and gutters.
· Additional services are necessary to remove and replace additional areas of deteriorated curb.
· The original appropriations of $75,000 of Motor Fuel Tax funds need to be supplemented by $20,000 to cover cumulative change orders which exceed 10% of the original contract.

R E S O L U T I O N

WHEREAS, in order to preserve pavement condition of the concrete pavements throughout the County highway system, each year the Division of Transportation undertakes the concrete pavement patching, where necessary, on various County highways; and

WHEREAS, this Lake County Board, by prior resolution at its meeting of February 14, 2012, appropriated $50,000 of Motor Fuel Tax funds for the improvement of the aforesaid section; and

WHEREAS, this Lake County Board, by prior resolution at its meeting of May 8, 2012, appropriated a supplemental amount of $25,000 of Motor Fuel Tax funds for the improvement of the aforesaid section; and

WHEREAS, additional services are necessary to remove and replace additional areas of deteriorated curb, and a supplemental appropriation of $20,000 is required; and

WHEREAS, the Lake County Purchasing Ordinance requires the Public Works and Transportation Committee, and the Finance and Administrative Committee to approve change orders with cumulative totals above 10% of the original contract.

NOW, THEREFORE BE IT RESOLVED by this Lake County Board, that a supplemental appropriation of $20,000 of Motor Fuel Tax is hereby authorized for the 2012 Concrete Pavem...
