Staff Summary
* The Health Department received $74,642 in additional funding as a result of increased reimbursement rates from the Illinois Department of Healthcare and Family Service to help support the sustainability of the Pathways to Success Care Coordination and Support Organization program.
* This funding is to support the program for Medicaid enrolled youth under the age of 21 who have complex, high-risk behavioral health needs and require evidenced-informed intensive care coordination along with home and community-based services to supplement existing treatment interventions.
* This will fund one new position for a Care Coordinator, computer equipment, contractual services, and supplies.
* If funding for the positions and services end, and new funding is not secured, the positions and services will be eliminated.

RESOLUTION
WHEREAS, the Lake County Health Department and Community Health Center has received notice of increased reimbursement rates from the Illinois Department of Healthcare and Family Services to help support the sustainability of the Pathways to Success Care Coordination and Support Organization program (CCSO); and
WHEREAS, these dollars will provide an increase in services to Lake County with no impact on local taxes; and
WHEREAS, this increase will be used for one new position, Care Coordinator, that will be eliminated if or when this funding is no longer available and no other funding source outside of property tax dollars has been identified; and
WHEREAS, the additional funding amount totaling $74,642 for the period of December 1, 2025, through November 30, 2026, has not been previously appropriated, having been approved after adoption of the Fiscal Year 2026 budget; and
