As a federally qualified health center (FQHC), who applies annually for Federal Tort Claims Act (FTCA) malpractice insurance, the Lake County Health Department and Community Health Center (LCHD/CHC) is required to hold a Clinical Risk and Patient Safety Committee meeting on a quarterly basis to address clinical risk and patient safety identified areas of concern. HRSA is now requiring that we conduct a quarterly risk assessment focused on risk identification, risk analysis, risk evaluation, and action planning. The risk assessment template has been developed to meet this requirement and will be conducted and presented to the Clinical Risk and Patient Safety Committee on a quarterly basis.
