Staff Summary
· Request for Proposals were called for by publication for Pharmaceuticals and Related Services for Winchester House.
· Requests for Proposals were extended to ten (10) firms and six (6) sealed proposals were received and opened on June 24, 2010.
· All responses were reviewed and two (2) finalists were interviewed.
· Based on the criteria set forth in the Request for Proposal, and the interviews, Winchester House Administration selected Forum Extended Care Services, Skokie, IL as the most favorable proposal for Lake County.
· This contract will cost the County an estimated annual amount of $350,000 charging $338,000 to account 218-4102010-62040-000-000-000-00000 and $12,000 to 218-4102010-71150-000-000-000-00000.

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, Request for Proposals were called for by publication for Pharmaceuticals and related services; and
WHEREAS, the services include the provision of prescription and non-prescription drugs, pharmacy consulting services and direct billing to all third-party payers for eligible residents; and
WHEREAS, subsequent to said publication, Request for Proposals were extended to ten (10) firms and sealed proposals were received and opened on June 24, 2010 from six (6) firms; and
WHEREAS, Winchester House Administration reviewed all responses and held interviews with two (2) firms; and
WHEREAS, based on the interviews and the criteria set forth in the Request for Proposal, Winchester House Administration selected Forum Extended Care Services, Skokie, IL as the most favorable proposal for Lake County; and
