Staff Summary
· The Lake County Coroner’s Office receives an annual grant from the Death Certificate Surcharge Fund provided by the Illinois Department of Public Health for the purposes of training personnel, purchasing equipment and the implementation of an electronic database for death certificates.
· The funds for this grant are provided by local registrars, who, by law, must deposit a fee of $2.00 for each certified copy of a death certificate and fetal death certificate.
· The Lake County Coroner’s Office wishes to deposit the amount of this grant, $4,415 in account number 101-3500010-45333-000-000-000-3526N.
· The Lake County Coroner’s Offices requests an emergency appropriation in the amount of $4,415 in account number 101-3500010-71500-000-000-000-3526N.

WHEREAS, Public Act 93-0045, in relation to Public Health, Section 5, The Vital Records Act and Section 25.5, The Death Certificate Surcharge Fund, requires county clerks and local registrars to charge a $2.00 fee for each certified copy of a death registration, in addition to any other fees charged by county clerks and local registrars; and

WHEREAS, Public Act 93-0045, in relation to Public Health, Section 5, The Vital Records Act and Section 25.5, The Death Certificate Surcharge Fund provides that the $2.00 fee must be transmitted to the State Registrar monthly and deposited into the Death Certificate Surcharge Fund for the purpose of implementing a statewide electronic reporting system for death registrations and for other purposes; and

WHEREAS, 410 ILCS 535/25.5, The Death Certificate Surcharge Fund is a Grant provided to coroners offices for the purposes of training coroners and deputy Coroners by the Illinois Law Enforcement Training Standards Board for homicide investigation and for other ...
