IlliniCare, Health Grant - $162,500 - Grant Approval - Burke

Lake County Health Department and Community Health Center (LCHD/CHC) received Health Grant funding from IlliniCare in the amount of $162,500. This will fund one new part-time Physical Health Nursing Specialist position, computer, supplies, and mileage reimbursement. This position will be responsible for identifying, implementing and ensuring sustainability of workflows to improve the provision of care as related to value-based care operations. In addition, this position will be responsible for data collection, data validation, and accuracy of UDS and HEDIS reporting and identifying, implementing, and ensuring sustainability of workflows related to these quality metrics and their improvements.

To approve the IlliniCare, Health Grant funding and emergency appropriate $162,500 for FY24.
