Staff Summary
• Lake County, as mandated by the US Department of Housing & Urban Development (HUD), operates a Homeless Management Information System (HMIS).
• The HMIS is used to track clients throughout the broader Continuum of Care and coordinate the provision of services. Lake County uses ServicePoint for its HMIS.
• ServicePoint has recently been expanded to meet two community needs: 1) “Coordinated Entry” into the homeless services system in Lake County of people who are chronically homeless and in need of permanent supportive housing; and 2) Referral tracking and coordination between service providers with clients in Lake County’s Behavioral Health Treatment Court.
