Presented are two (2) LIP for initial appointment. The Medical Director has verified the privileges requested by the provider are appropriate for the provider’s training and experience. The Medical Director and the Governing Council Personnel Committee have studied the applications, raised questions where necessary, and recommend approval.

To approve the initial appointment of the two (2) licensed independent practitioners, as presented.
