WHEREAS, Member Michael Welch has resigned from the Lake County Board of Health; and

WHEREAS, Michael Welch’s resignation has created a vacancy in the Lake County Board of Health; and

WHEREAS, your Chair requests that this County Board confirm the appointment of Frank Belmonte as a member of the Lake County Board of Health to complete the vacated term ending on June 30, 2019.

NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the appointment of Frank Belmonte as member of the Lake County Board of Health to complete the vacated term ending June 30, 2019 be, and the same is hereby confirmed.
