Presented are four (4) LIPs for reappointment. The proper verifications have been carried out and there were no identified issues. The Medical Director has verified the privileges requested by the providers are appropriate for the providers’ training and experience. The Medical Director, the Board of Health Credentialing Committee, and the Governing Council Personnel Committee has studied the applications, raised questions where necessary, and based upon satisfactory responses, recommends approval of the request for reappointment.
