Presented are (5) LIPs for reappointment. The Medical Director has verified the privileges requested by the provider are appropriate for the provider’s training and experience. The Medical Director, the Board of Health Credentialing Committee, and the Governing Council Personnel Committee have studied the applications, raised questions where necessary, and recommend approval.
