Lake County Health Department and Community Health Center (LCHD/CHC) received notification from the Illinois Department of Healthcare and Family Services that increased reimbursement rates were approved in December 2024 to help support the sustainability of the Pathways to Success Care Coordination and Support Organization program (CCSO). The funding amount to be appropriated is $184,167 for LCFY25. The CCSO is a program for Medicaid enrolled youth under the age of 21 who have complex, high-risk behavioral health needs and require evidenced-informed intensive care coordination along with home and community-based services to supplement existing treatment interventions. The goal of the program is to closely coordinate care across all programs and systems working with the youth and family; improve access to crisis supports and community-based services; minimize unnecessary use of inpatient psychiatric hospitalization and emergency rooms; improve family functioning and create a more stable living situation; improve school attendance and performance; reduce contact with law enforcement and child welfare; and enhance clinical, functional, and cost outcomes. To maintain compliance with the Illinois Department of Health Care and Family Services and the CCSO model, strict staff to client ratios and supervisor to staff ratios must be retained. This will fund three new positions, computer equipment, contractual services, and supplies.
