Licensed Independent Practitioner (LIP) Reappointment - Burke

Presented are three (3) LIP for reappointment. The proper verifications have been carried out and there were no identified issues. The Medical Director has verified the privileges requested by the provider are appropriate for the provider’s training and experience. The Medical Director, the Board of Health Credentialing Committee, and the Governing Council Personnel Committee have studied the applications, raised questions where necessary, and based upon satisfactory responses, recommends approval of the request for reappointment.
