Scope of Project Adjustment, Form 5A: Services and Service Delivery Methods - Burke

As a Federally Qualified Health Center, the Lake County Health Department and Community Health Center (LCHD/CHC) is required to maintain a scope of project with the Health Resources and Services Administration (HRSA). The scope defines the approved services, health centers, and service activities that are served with section 330 grant funds. LCHD/CHC is responsible for updating this scope of project as organizational services change.
The scope adjustment needs to be approved by the Governing Council. If Governing Council approval is obtained, LCHD/CHC will submit a formal adjustment in scope request to HRSA.
Form 5A defines required, additional and specialty services delivered via the following service delivery methods:
• Column I services are provided directly by LCHD/CHC
• Column II services are provided contractually with LCHD/CHC paying for the services
• Column III services are provided through a contractual, referral relationship where LCHD/CHC does not pay for the service.
