SPECIAL RECOGNITION
WHEREAS, the Lake County Health Department and Community Health Center began serving patients as a designated Federally Qualified Health Center (FQHC) Look-Alike in September 1995, and in May 1998 became a Bureau of Primary Health Care Section 330(e) funded Community Health Center; and
WHEREAS, the Lake County Health Department and Community Health Center has spent 30 years expanding access to high-quality care and providing services to all who live, work, and enjoy Lake County; and
WHEREAS, the Lake County Health Department and Community Health Center FQHC began with a single location and has since expanded to nine locations throughout Lake County; and
WHEREAS, these FQHC locations now offer 13 programs, including Medical, Dental, Prenatal, Family Planning, Gynecology, Women Infants and Children (WIC), Illinois Breast and Cervical Cancer Program (IBCCP), Need2 Know (STI services), Psychiatry/Behavioral Health Advanced Practice Registered Nurse (APRN) Services, Behavioral Health Therapy Services, Laboratory Services, Insurance Navigation Services and Community Health Worker Services; and
WHEREAS, this care is essential to the community's most vulnerable and underserved populations, serving 32,975 unique patients with 113,189 patient visits in 2024; and
WHEREAS, today, the Lake County Health Department and Community Health Center FQHC has over 450 dedicated and compassionate professionals who deliver critical health care and support services to their patients; and
WHEREAS, the Lake County Health Department and Community Health Center is proudly celebrating its 30th anniversary as a mission-driven, Federally Qualified Health Center, and the largest health service provider in Lake County.
