FY2021 HRSA American Rescue Plan Act Funding for Health Centers (H8F) $8,760,875.00 - Grant Budget Revision - Burke

On March 26, 2021, the Health Resources and Services Administration (HRSA) awarded Lake County Health Department and Community Health Center (LCHD/CHC) ARPA Funding for Health Centers grant. HRSA ARPA funds are to be used for expenses related to the following activities:
* COVID-19 vaccinations
* COVID-19 response and treatment capacity
* Maintaining and increasing health center capacity
* Recovery and stabilization.
LCHD/CHC was awarded $8,760,875.00 for the project period April 1, 2021 - March 31, 2023. LCHD/CHC is requesting a budget revision to our ARPA Funding for Health Centers grant due to barriers in our ability to hire staff and purchase the approved budget items within the project period.
LCHD/CHC proposes the following changes in the ARPA budget:
* Personnel - $4,405,601.00
o Proposed new total: $2,088,152.00 (Reduction of $2,317,449.00)
* Fringe - $1,730,806.00
o Proposed new total: $708,858.00 (Reduction of $1,021,948.00)
* Travel - $0
o Proposed new total: $5,000.00
* Equipment - $37,945.00
o Proposed new total: $203,873.00 (Increase of $165,928.00, adding new equipment)
* Autoclave with quiet door with printer (Women's Health) (9)
* Nanosonic Trophon Probe - high level disinfection for ultra-sound machine (Women's Health) (1)
* Eye Pac Crystal Vue Retinal Camera NFC 700 Robotic Fundus Camera with extended warranty and shipping and handling (7)
* Dental Adult Sensors (5)
* Supplies - $90,243.00
o Proposed new total: $664,148.00 (Increase of $573,905.00)
* Vitals monitors with stands (4)
* Vaccine freezer replacements (8) and refrigerator replacements (5)
* Chicago Biomed Dental x-ray machines (6)
* Computer arms (12)
* Computer: Teguar medical grade all-in-one computers (93)
* Diabetes initiative: FreeStyle Libre for 107 patients
