Lake County Health Department and Community Health Center (LCHD/CHC) is seeking a qualified firm to provide dental lab services to the health center dental program.
On April 18, 2024, a legal notice was published in The News Sun soliciting Requests for Proposals (RFP 24302) and the RFP was posted to the LCHD/CHC website. Eleven vendors registered for the RFP and three submitted sealed proposals to the Materials Management office on May 1, 2024.
An evaluation committee comprised of LCHD/CHC staff reviewed the proposals, submitted evaluation scoresheets, and determined that Ottawa Dental Laboratory of Ottawa, Illinois, best meets the needs of LCHD/CHC. Ottawa Dental Laboratory is the current provider of these services and LCHD/CHC has a positive experience with this company.
Suggested Motion
To approve the award for RFP #24302 Dental Lab Services for the Lake County Health Department and Community Health Center in the estimated annual amount of $80,000 to Ottawa Dental Laboratory of Ottawa, IL, and authorize the Executive Director or designee to execute the necessary negotiated documents.
