New Client Wait List
As of September 25, 2024, there are 1,569 individuals on our general medicine new client waitlist, 1,504 adults and 65 pediatrics. These individuals are looking to establish care in our following health centers:
• Grand: 39%
• Belvidere Medical Building: 37%
• Zion: 9%
• Midlakes: 7 %
• North Shore: 4%
• Libertyville: 3%
• North Chicago: 1%
The total new client wait list has been decreased by 279 total individuals, 54 adults and 225 pediatric clients were removed from the waitlist since the September 3, 2024, report to Governing Council. There are currently 135 new patient appointment slots per week.

Federal Tort Claims Act (FTCA) Application for 2025
The FTCA application for malpractice coverage for the FQHC for Calendar Year 2025 was completed and submitted on June 24, 2024. On August 14, 2024, HRSA emailed LCHD/CHC requesting additional information. The additional information was submitted on September 13, 2024.
