Lake County Health Department and Community Health Center received grant funding through the AIDS Foundation of Chicago for Ryan White Part A services in the amount of $196,203. The period of performance is from March 1, 2025, through February 28, 2026. $147,152 will be appropriated in LCFY25, and the difference was accounted for in the LCFY26 budget. The purpose of this funding is to support medical and non-medical case management staff who provide care coordination, service linkage, and support services for individuals living with HIV to improve health outcomes and retention in care. This will continue to fund three positions, supplies, and telecommunications.

To approve the AIDS Foundation of Chicago Ryan White Part A grant funding of $196,203 and emergency appropriate $147,152 for FY25.
