Presented are twelve (12) LIP for appointment, reappointment, approval of privileges, or privilege revision. The Medical Director has verified the privileges requested by the provider are appropriate for the provider’s training and experience. The Medical Director and the Personnel Committee have studied the applications, raised questions where necessary, and recommend approval.
