Illinois Department of Healthcare and Family Services, Care Coordination and Support Grant, $625,000 - Grant Approval - Johnson

Lake County Health Department and Community Health Center (LCHD/CHC), through the Illinois Department of Healthcare and Family Services for the Care Coordination and Support program, received an amendment to extend the funding period to March 31, 2025, with additional funds in the amount of $625,000. The funding period is now October 1, 2022, to March 31, 2025, and the overall funds for this program total $1,000,000. The goal of the program is to enhance care coordination across programs, improve access to crisis supports and community-based services for those with behavioral health needs, minimize unnecessary use of inpatient psychiatric hospitalization and emergency rooms, and boost clinical, functional, and cost outcomes. The funds will continue to support existing staff, a new Care Coordinator, a new Community Relations Specialist, supplies, contractual services, and computer equipment.

To approve the Illinois Department of Healthcare and Family Services, Care Coordination and Support grant additional funding and emergency appropriate $625,000 for FY24.
